Ask them! Look for the adams apple. Bigger hands, taller, broad shouldered, deeper voice, over exaggerated female mannerisms. Some will have a scarf around their necks to hide the adams apple. If you are looking for them for a date keep in mind that they are men who can be very aggressive. They have been known to travel in packs and are involved in drugging and robbing gullible tourists.

The simple answer would be: if you can’t tell, why would you care?
But if you’re really ladyboy-phobic, and your ladyboy-dar does not go off, then watch out for those ladies who appear excessively effeminated AND aggressive. Those are usually good behavioral signs to spot ladyboys; but remember, most of bar girls are aggressive, but they will not be excessively effeminated.
